There is no known interaction between clarithromycin and paracetamol. It is generally safe to take these two medicines together.
Why this happens
These two drugs are processed by different pathways in the body. Clarithromycin is primarily broken down by a specific liver enzyme (CYP3A4) which it also inhibits, while paracetamol is processed through different liver pathways (glucuronidation and sulfation). Because they do not compete for the same 'machinery' in the liver, they do not interfere with each other.
What you should do
You can take paracetamol while you are on a course of clarithromycin. Always follow the dosage instructions on the packet for paracetamol and complete the full course of your antibiotic as prescribed by your doctor. If you feel unwell or develop a rash, contact your healthcare provider.
Important precautions
Heart rhythm disorders (Long QT syndrome)
Clarithromycin can cause serious heart rhythm problems. You must not take it if you have a history of 'Long QT syndrome' or are taking other medicines that affect your heart rate.
Liver disease or chronic alcohol use
If you have liver problems or drink large amounts of alcohol regularly, you are at a much higher risk of fatal liver damage from paracetamol, even at standard doses.

Alcohol
While there is no direct interaction, both clarithromycin and alcohol can cause side effects like nausea, dizziness, or stomach upset. It is generally best to avoid alcohol until you have finished your course and feel better.
