Can I take Folitrax and Hqtor together?

Taking Folitrax (methotrexate) and Hqtor (hydroxychloroquine) together can increase the risk of side effects, particularly those affecting the liver. However, these two medicines are very commonly prescribed together by specialists to treat conditions like rheumatoid arthritis. While they work well as a team to control inflammation, they require careful monitoring.

Why this happens

Both medicines are processed by the liver. When used together, they can place extra strain on the liver, potentially leading to higher levels of liver enzymes in the blood. Additionally, both drugs can affect your blood cell counts, though hydroxychloroquine is much less likely to do so than methotrexate.

You can take these medicines together, but you must attend all scheduled blood tests. Your doctor will use these tests to check your liver function and blood cell counts. If you notice yellowing of the skin or eyes (jaundice), dark urine, or unusual bruising and tiredness, contact your doctor immediately. Avoid drinking alcohol, as this significantly increases the risk of liver damage when taking Folitrax.

Pregnancy and conception

You must not take this medicine if you are pregnant or planning to become pregnant, as it causes severe birth defects and miscarriage. Both men and women must use effective contraception during treatment and for at least 6 months after stopping.

Infection and blood disorders

This medicine can severely suppress your immune system and bone marrow. You must seek immediate medical attention if you develop a fever, sore throat, mouth ulcers, or unexplained bruising/bleeding.

Vision changes

This medicine can cause permanent damage to the retina (the back of the eye). You must have regular eye tests and report any blurred vision or changes in your sight immediately.

Alcohol

You must strictly avoid or significantly limit alcohol while taking Folitrax. Both methotrexate and alcohol can cause liver damage; combining them significantly increases the risk of serious liver toxicity and scarring (cirrhosis).

Caffeine (Coffee, Tea, Cola)

Large amounts of caffeine may reduce the effectiveness of methotrexate in treating conditions like rheumatoid arthritis. While moderate intake is usually fine, try to keep your caffeine consumption consistent and avoid excessive amounts.

Grapefruit and Grapefruit Juice

Grapefruit can increase the levels of hydroxychloroquine in your blood, which may increase the risk of side effects. It is best to avoid grapefruit products while taking this medicine.
