Can I take Fluvoxamine and Olanzapine together?

Taking fluvoxamine and olanzapine together can significantly increase the amount of olanzapine in your bloodstream. This makes you much more likely to experience side effects such as extreme sleepiness, dizziness, weight gain, dry mouth, constipation, and a higher risk of movement disorders (shaking or stiffness). In some cases, it can also affect your heart rhythm or cause a sudden drop in blood pressure when standing up.

Why this happens

Your liver uses a specific protein (enzyme) called CYP1A2 to break down olanzapine and remove it from your body. Fluvoxamine is a very strong 'inhibitor' of this protein, meaning it blocks the liver's ability to clear the olanzapine. This causes the olanzapine to build up to potentially toxic levels.

You must speak to your doctor before taking these two medicines together. If they are prescribed together, your doctor will usually need to prescribe a much lower dose of olanzapine than normal (often one-third of the usual dose). If you are already taking both and feel excessively drowsy, dizzy, or faint, contact your healthcare provider immediately.
